Description

The Ohio Developmental Disabilities Council wishes to support one-time initiatives, not otherwise included in the state plan, in an expeditious manner without the necessity of waiting for the next meeting. This would be accomplished by sponsorship/collaboration on a variety of events/conferences as they relate to significant issues in the developmental disabilities arena.

Expenditures shall further Council's positions and its efforts to support persons with developmental disabilities and their families to become more fully integrated into their communities, to learn more about various disabilities and the most-up-to-date means to support a person with such a disability, and to foster greater public awareness of both disability issues and Council's role in promoting systems change.

Expenditures shall complement and not conflict with Council's State Plan on Developmental Disabilities.

Instructions

Applicants shall submit a written request for funds that clearly indicates the purpose for which the funds are being requested. Expenditures from the fund shall be in the form of mini-grants to support an individual or an organization. Each recipient will be asked to provide matching funds either in cash or in kind, and to submit an expenditure report. Recipients will also be asked to return all unexpended funds in a check made payable to the Treasurer, State of Ohio.

Discretionary Fund requests in excess of $2,000 will be submitted to the Ohio Developmental Disabilities Council Executive Committee for consideration and determination. Requests for more than $5,000 will be submitted to the full Council for consideration and determination. Any requests that require full Council approval must be received ten (10) calendar days prior to the Council meeting. Please see our website at www.ddc.ohio.gov for Council meeting dates.

Anyone requesting funds from the Discretionary Fund must complete this application. All applications must include a description of the activity you are seeking funds for. Applicants must explain, in their description, how the un- and underserved population will be reached, specifically, which groups will be targeted and what strategies will be employed. A detailed budget narrative that explains how federal funds will be spent and matching funds provided should be included as an attachment. Please refer to the Grantee Guidelines for a description of what should be included in the Budget Justification. Applicants are also encouraged to attach documentation that supports their request.

Applicants seeking more than one discretionary fund request within the federal fiscal year are required to contact the DD Council office prior to submitting application. The attached Assurances must accompany the application.

Minimum Requirements

There are no restrictions on the type of entity that may apply for this project.

All applications must include proof of non-profit status (if the applicant is a non-profit organization), and a signed copy of the Council's Assurances form, which is attached. These materials can be uploaded as part of the application submission in DD Suite.

All applicants, with the exception of individuals or an entity exempted under 2 CFR 25.110 (b) or (c), or has an exception approved under 2 CFR 25.110 (d), must be registered in the System for Award Management (SAM) before submitting their application and must include a valid Dun and Bradstreet Universal Number System (DUNS) number in the application. Applicants are also required to maintain an active SAM registration with current information at all times that an application is pending or the applicant holds this federal award. The Ohio Developmental Disabilities Council may not make an award to an applicant until the applicant has complied with the requirements of DUNS and SAM. Please refer to the DUNS AND SAM REGISTRATION section of the Grantee Guidelines for detailed instructions.

State Plan Language - This is a complete description of the project which includes intended outcomes and proposed performance measures.

DD Council Assurances - These assurances must be signed and attached as part of the application submission. Refusal to include a completed DD Council Federal Assurances form will result in the application being removed from consideration.
